We validate the first 40 minute, after that, building parking fee is $2.00 for 15min, each additional 15min $2.00.
The Metered parking on the street right in front of the building and Free Street Parking on 25th street by the Clover Park and around.

We accept some of the major insurances and Worker’s Comp.
Anthem Blue Cross, Blue Shield, United Healthcare, Motion Picture Industry Health Plan, SAG,DAG, Cigna, and so on.

Cancellation Policy
Please understand your appointment time is reserved for you. We recognize there may be occasions when you need to cancel or reschedule an appointment. If you need to make any changes to your appointment for any reason, we require a minimum of 24-hour advanced notice to avoid cancellation/rescheduling fees in the amount of your scheduled visit.
