Parking
The building parking fee is $2 for 15 minutes. The metered parking on the street on 23rd Street. Street parking 24th St. and Arizona, Chelsea and Arizona Ave between Arizona and Santa Monica, Broadway between 23rd and 20th St.
Insurance
We are an out-of-network provider for all the insurance. Please call your insurance and ask what is your out-of-network acupuncture benefit, and you can find out your coverage. We provide superbill for your insurance claim, and for some insurances, we submit the claims for the patients.
Patient Forms
If you print out the forms and fill them out before the visit or save to your computer and email it to the office([email protected]), which will save your waiting time.
Cancellation Policy
Please understand your appointment time is reserved for you. We recognize there may be occasions when you need to cancel or reschedule an appointment. If you need to make any changes to your appointment for any reason, we require a minimum of 24-hour advanced notice to avoid cancellation/rescheduling fees in the amount of your scheduled visit.
